Dialog+
What is DIALOG+?
DIALOG+ is a patient led outcomes scale whereby people entering secondary mental health services are asked to rate their satisfaction around quality of life and needs for care on 13 areas:
- Your mental health
- Your physical health
- Your job situation
- Your accommodation
- Your leisure activities
- Your partner/family
- Your friendships
- Your personal safety
- Your medication
- The practical help you receive
- Your consultations with mental health professionals
- Your finances
- Your alcohol / substance abuse
DIALOG+ is a facilitated conversation focusing on the lowest scored areas using the DIALOG scale. The Service User and Health Care Professional work together in a strength-based therapeutic way, co-producing a holistic care plan.
Why use DIALOG+?
- DIALOG+ enables proactive, personalised conversations which is led by the Service User with their Health Care Professional ,supporting self-management and co-production. This supports Service Users to move forward on their individual journey of recovery.
- DIALOG+ is being used to help inform the redesign of the care planning processes within mental health services.
As a minimum, it is recommended that DIALOG+ is completed at the start of treatment, during review dates and routine appointments and at the end of treatment.
